Downloads attached. The text boxes in the AT37 tend to misbehave each time the document is opened or moved, you might have to thrash them around a bit. The obsolete terminology has never caused any issues in my experience – it’s basically an input sheet to allow the Tribunal clerks to create a record on their case management system, as long as you give them the data items they need they are happy.
