Evaluation of benefit take-up exercises

    Hi

    I was wondering if anybody out there has a document/ template that they use for evaluating benefit take-up exercises. The sort of thing I’m looking at is :

    How was the take-up exercise identified
    How much staff resource expended on it
    How much ‘partner/stakeholder ‘ resource was required
    How many members of the public contacted/ spoken to (obviously depends on how the exrcie was done, was it an ‘event’ or a desk-based exercise)
    How many claims subsequently received
    How much extra benefit awarded
    Does the council consider this exercise value for money, and reasons for that decision
    Would the council repeat the exercise
    Lessons learned

    There are probably other considerations I haven’t yet thought of.

    If anyone has already got such a document, and it works, I’d be grateful if you could share it. I couldn’t see anything in the ‘library’ section.

    I’ve “risk assessment” template which I use for strands of take up work and you are very welcome to use or hack about as you wish.
