Shortened claim form for ‘starting work’

    Anyone use a shortened claim form to collect details where someone moves into employment – i.e. collects capital details and seeks to verify details relating to return to work

    Are you willing to send me a copy?

    we also have a shortened form that we have started using from this week (we now have so many shortened forms we don’t know what to do with them) but as our is designed in house could I please have a copy of yours in case I need to steal/borrow/plagiarise the best bits from it.

    Thanks Martin,

    Very different to ours which is A3 so effectively 2 sides of A4 to complete once you take out the front cover and declartion.

    We still have to send a letter confirming that the claim has been suspended so I think the potential for stealing is still there.

    Idea for the webmaster. Let’s have a library area in which we can save .pdf files of claim forms & leaflets etc then we can all save to it and dip in and take things out when we need to.

    Yes, we have been thinking along those lines and will launch a library section very soon. However, we prefer not to use PDF on the site since it is not easy to cut and paste what bits are wanted. This is why all the regs are in seperate individual files and not in one massive PDF version. Takes much longer to do though.

    Meanwhile, please send coopies to library@octagonconsultancy.com. PDF / WORD (or both if available) is fine.
